Do you accept my insurance?

We accept Blue Cross, Anthem, Cigna, United Healthcare, Aetna, & Tricare

My insurance isn't accepted, is there another option?

Yes! We do offer services even if your insurance is not accepted. If that is the case our rates are:

Therapist (Doris Bell): Intake $250 / Follow-up $150

Therapist (Gretchen Shay): Intake $150 / Follow-up $90

Therapist (Robyn Huckabee): Intake $75 / Follow-up $75

Nurse Practitioner: Intake $275 / Follow-up $100

Nurse Practitioner: Transfer Patient Intake $275 / Follow-up $100

* If a new patient does not have paperwork completed 48 hours before their appointment then the appointment may be cancelled and the patient is subject to a $100 fee in order to reschedule.

Are there any other fees/charges?

We do have fees for additional work outside of a patients appointment including:

Prescription Refill: $25

Controlled Substance Refill: $35

Forms: $50 (first request) / $35 (additional request(s))

Cancellation Fee (without 24 hr notice): $25

After Hours fee: $50 (appointments outside of normal business hours)

Additionally, in the event that a patient does no shows an appointment they may be subject to a no show charge of:


$60 - if seeing a Nurse Practitioner

$100 - if seeing a Therapist

I'm interested in setting up an appointment, how do I start the process?

Contact our front office by phone at 256-325-2500 or via e-mail at to start the process.

Afterwards, you will gain access to the patient portal and can begin filling out the required forms.

Do you offer telehealth?

Yes! Inform our front office that telehealth is your method of choice for treatment or discuss it with your provider, and we will accommodate your request.

I have a question for my provider outside of my appointment, how do I get in contact?

If you need immediate assistance or have a medical emergency, please dial 911.

Otherwise, you can reach your provider by sending a message via the patient portal, sending an e-mail to, or leaving a voicemail via phone at (256)325-2500. Due to volume, please give 48 hours to receive a response.

I am unable to pay at this time, do I have any options?

We understand that hardships may come up in everyday life, and our goal is never to prevent anyone from getting the care they need.
